How you prepare for Jaw Surgery

In most cases, an orthodontist places braces on your teeth before orthognathic surgery. Braces are usually on for 12 to18 months before surgery to level and align your teeth in preparation for surgery.

Your orthodontist and oral and maxillofacial surgeon work together to develop your treatment plan. X-rays, pictures and models of your teeth are part of the planning for your jaw surgery. Occasionally, the difference in the way teeth fit together will require either reshaping of the teeth, covering the teeth with crowns or both to complete correction.

Types of jaw surgery

Upper Jaw Surgery

Maxillary osteotomy / Upper jaw surgery

Maxillary osteotomy (Upper Jaw Surgery) is the surgery that’s done on your upper jaw (Maxilla).  

Conditions that may call for maxillary osteotomy include:

  • an upper jaw that protrudes or recedes significantly
  • an open bite, which is when your back teeth (molars) don’t touch when your mouth is closed
  • a crossbite, which is when some of your bottom teeth sit outside of your upper teeth when your mouth is closed
  • midfacial hyperplasia, which is a condition where growth in the middle portion of your face is reduced

Procedure overview

During this procedure, your surgeon will:

  1. make an incision in the gums above your upper teeth, allowing them to access the bones of your upper jaw
  2. cut into the bone of your upper jaw in a way that allows them to move it as a single unit
  3. move this portion of your upper jaw forward so that it aligns and fits properly with your lower teeth
  4. place plates or screws to hold the adjusted bone in its new position
  5. use stitches to close the incision in your gums

Mandibular osteotomy (Lower Jaw Surgery) refers to surgery that’s performed on your lower jaw (mandible).

A mandibular osteotomy can correct:

Procedure overview

When you have a mandibular osteotomy, your surgeon will:

  1. make an incision into your gums on each side of your lower jaw, just behind your molars

  4. place plates or screws to hold the adjusted jawbone in its new position

Bimaxillary osteotomy (Bi jaw Surgery) is the surgery performed on both your upper and your lower jaw. It’s done when a condition affects both jaws.

  1. make an incision into your gums around your lower lip
  2. cut part of the chinbone, which allows them to move it
  3. carefully move the chinbone into its new position
  4. place small plates or screws to help hold the adjusted bone in its new position
  5. close the incision with stitche

What can I expect before, During and after Jaw surgery?

Before Jaw Surgery

  • Consultation with Oral and Maxillofacial surgeon and orthodontist.
  • Investigations and planning – Takes 4-5 days for surgical planning and preparation of splints and models.
  • Blood tests and reserving blood for blood transfusion.
  • Admission on the day of surgery.

During the procedure

  • Surgery 2-6 hours under general anaesthesia.
  • Surgery usually can be performed inside your mouth, so no facial scars show on your chin, jaw or around the mouth. However, sometimes small incisions may be required outside your mouth.
  • Your surgeon makes cuts in the jawbones and moves them into the correct position. 
  • Once your jaw movement is completed, tiny bone plates, screws, wires and rubber bands may be used to secure the bones into their new position. These screws — which are smaller than a bracket used for braces — become integrated into the bone structure over time.
  • In some cases, extra bone may be added to the jaw. Your surgeon transfers the bone from your hip, leg or rib and secures it with plates and screws. In other cases, bone may be reshaped to provide a better fit.
  • Jaw surgery may be performed on the upper jaw, lower jaw, chin or any combination of these.

After Surgery

  • Shifter to recovery room 2-3 hours.
  • One night ICU stay which might also involve blood transfusions. ICU stay is for monitoring of the airway and breathing issues.
  • Shifted to ward and ward stay for 2-3 days.
  • After your surgery, it’s normal to experience swelling, stiffness, and discomfort in your face and jaw. These should go away over time. 
  • In the meantime, your doctor will prescribe medications to help ease these symptoms.
  • Rubber bands applied to braces.
  • Discharge in 3-5 days
  • Soft or semi-solid diet for 3 weeks.
  • Follow up with Oral and Maxillofacial Surgeon and orthodontist. 
  • Return to normal activities after 1 week. Strenuous physical activity after 6 weeks.
  • After several weeks of recovery, your orthodontist will continue aligning your teeth with braces.
  • When your braces are removed, your orthodontist will give you a retainer to help keep your teeth aligned.

Care after the Jaw surgery

  • We encourage you to start drinking and eating soft foods after the surgery. 
  • Most patients lose some weight after surgery, so it is important you make an effort to keep up your calories. 
  • Your mouth opening will be reduced after surgery owing to swelling, so you should eat soft foods (soups, smoothies, yogurt etc.)
  • Over the next few weeks you should return to a normal diet, avoiding hard foods for up to six weeks whilst the jaw bones heal.
  • Tooth brushing is very important throughout treatment but especially so after surgery. You are advised to use a soft toothbrush for the first few weeks after surgery. 
  • We will give more detailed practical advice before surgery and also before you leave the hospital to go home. We usually review patients weekly after the surgery for 2 to 3 weeks.
